Steps to create a custom paper size for printing from the Print menu in Mac OS X:
Load custom paper in the input tray.
Open the file you want to print.
Click File, then click Print.
From the Paper Size drop-down menu, select Manage Custom Sizes.
Click the plus sign to create a new paper size.
Double-click the default Untitled name, then name the new custom size.
Next to Paper Size, fill in the dimensions of your customer paper size.
Click the Paper Size drop-down menu and select the new custom paper size.
Change any desired print settings, then print.
